Important – Please Read: The following Ph413 Reward Card Agreement describes the terms and conditions that apply to the program. By using your Ph413 Reward card, you, the cardholder, agree to the terms of this agreement. Please keep a copy of this agreement for your records.

General Terms: The Ph413 Reward card is issued to you by Ph413, Inc. It allows your designated worship facility (Beneficiary)*, to receive points that can only be redeemed towards purchases at any Ph413, Inc. affiliated music store.  For a current list of affiliated stores please visit www.ph413.com. Accumulated points will only be redeemable by the Beneficiary designated by you at each point of sale at the time of purchase (Beneficiary).  You can designate up to one worship facility per each purchase transaction. No credit card or credit line is associated with this rewards card.  Unless otherwise required by law, any value on the card for credits or points is non-refundable and cannot be redeemed for cash. You can check the balance of credits or point values at any time by visiting www.ph413.com. We reserve the right to correct the balance of credits or point values if we believe a clerical, billing or accounting error has occurred. If you have any questions regarding an error, we will investigate to determine if any adjustment is necessary.  If your card is lost or stolen, a new card may be issued and the balance of points or coupon value transferred. Ph413, Inc. is not responsible for unauthorized use of your reward card. Please report any unauthorized activity and a new card will be issued.

Benefits include: Purchase or credit points applied to the account of the designated Beneficiary throughout the year to be applied as a credit towards the purchase of goods by your designated Beneficiary.

Program: The Beneficiary designated at time of each purchase will receive 1 point for each $50.00 of merchandise spent with any affiliated music store at any of our locations as listed on www.ph413.com.  Points begin accumulation from the date of issuance of your Ph413 Reward Card.

For each point accumulated by the customer the designated Beneficiary will automatically receive a credit on its designated account equivalent to $1.00 towards an additional purchase at any Ph413, Inc. affiliated music store.  Each full point accumulated will be deducted from your rewards point total and applied to the account of the designated Beneficiary.  The remaining balance or any fraction of a point will remain.

The Ph413 Rewards Card terms and conditions may be amended at any time without prior notification.  We will notify you of any changes to the program by email or written notification by mail.  It is the customer’s responsibility to keep Ph413, Inc. advised of your current email address or mailing address. We may suspend or terminate this program at any time without notice or liability.  Allow 2-3 weeks for issuance of any new Ph413 Rewards Card.

Memberships are not transferable.

This agreement shall be governed by and construed in accordance with the laws of the State of Connecticut notwithstanding any conflict of law rules.

*to qualify as a Beneficiary, the worship facility must have the meaning set forth in the IRS Code Section 509(a) (1) and/or Section 170(b) (1) (A) (i).
